Skilled Wound Care | Effective date: November 1, 2020 | Last updated: November 1, 2020

Skilled Wound Care ("SWC," "we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect when you visit www.skilledwoundcare.com (the "Site"), interact with our advertising on LinkedIn or other platforms, or contact us about our physician wound care services for skilled nursing and long-term care facilities, and how we use, share, and protect that information. By using the Site or submitting information to us, you agree to the practices described in this policy.

1. Who this policy covers

This policy applies to visitors to the Site, to facility administrators, directors of nursing, clinicians, and other professionals who contact us or respond to our marketing, and to anyone who submits a form to us through the Site, LinkedIn, or another advertising platform. It does not govern the medical information of patients treated by our physicians in nursing facilities; that information is protected health information handled under HIPAA and our Notice of Privacy Practices, which is provided at the facility level. Please do not submit patient information through the Site or through any marketing form.

2. Information we collect

Information you give us. When you complete a contact form, request a consultation, download materials, register for an educational program, or apply for a position, we collect the information you provide, which may include your name, job title, facility or organization name, work email address, work phone number, city and state, and the content of your message.

Information from LinkedIn Lead Gen Forms. When you respond to one of our LinkedIn advertisements and submit a LinkedIn Lead Gen Form, LinkedIn provides us with the information you agree to share, which may include your first and last name, email address, phone number, job title, company name, and location, as pre-filled from your LinkedIn profile, together with any answers you type into the form.

LinkedIn's own handling of your information is governed by the LinkedIn Privacy Policy

You can review and manage the data you shared with us through your LinkedIn settings.

Information collected automatically. When you visit the Site we and our service providers automatically collect certain technical information, such as your IP address, browser type, device type, operating system, referring page, pages viewed, time spent, and approximate location derived from your IP address. We collect this through cookies, pixels, and similar technologies, including the LinkedIn Insight Tag and Google Analytics (see Section 6).

3. How we use your information

  • To respond to your inquiry, schedule a consultation, and provide information about our wound care program and services.

  • To communicate with you by email, phone, or mail about our services, educational programs, clinical updates, and events that may interest you as a facility leader or clinician.

  • To operate, maintain, and improve the Site and our marketing, including measuring the performance of our advertising on LinkedIn and other platforms.

  • To build and serve advertising to audiences of people who have shown interest in our services, including through retargeting on LinkedIn.

  • To process employment and contractor applications.

  • To comply with legal obligations, enforce our terms, and protect the rights, safety, and property of SWC, our physicians, our clients, and others.

We do not sell your personal information, and we do not use the information you submit through a marketing form for any purpose unrelated to the purposes listed above.

4. How we share your information

We share personal information only as follows:

  • Service providers. With vendors who perform services on our behalf, such as website hosting, customer relationship management (CRM) software, email delivery, analytics, and advertising measurement. These providers may use your information only to perform services for us.

  • Advertising platforms. With LinkedIn and similar platforms, in the form of hashed identifiers or aggregated data, to measure campaigns and to build audiences. We do not disclose the contents of your inquiry to these platforms.

  • Affiliates. With Skilled Facility Healthcare Solutions, Inc., the affiliated physician practice through which our physicians provide services, for the purposes described in this policy.

  • Legal and safety. When required by law, subpoena, or legal process, or when we believe disclosure is necessary to protect our rights or the safety of any person.

  • Business transfers. In connection with a merger, acquisition, financing, or sale of all or part of our business, subject to this policy.

We do not sell or rent your personal information to third parties, and we do not share it with other companies for their own marketing.

5. Marketing communications and your choices

If you submit a form or respond to our advertising, we may contact you about our services. You may opt out of marketing emails at any time by clicking the unsubscribe link in any email or by contacting us at the address in Section 11. You may ask us to stop calling you by telling us so during a call or by emailing us.

Opting out of marketing does not affect communications that are necessary to respond to a request you have made.

6. Cookies, analytics, and the LinkedIn Insight Tag

The Site uses cookies and similar technologies. Some are necessary for the Site to function. Others help us understand how visitors use the Site and measure our advertising. In particular:

  • LinkedIn Insight Tag. We use the LinkedIn Insight Tag, which allows LinkedIn to collect data about your visit (including URL, referrer, IP address, device and browser characteristics, and timestamp) so that we can measure the performance of our LinkedIn advertising and show ads to visitors who have shown interest in our services. LinkedIn does not share personally identifiable information with us; we receive only aggregated reports. LinkedIn members can control the use of their data for advertising in their LinkedIn settings and can opt out at https://www.linkedin.com/psettings/guest-controls/retargeting-opt-out.

  • Google Analytics. We may use Google Analytics to understand Site traffic. You can opt out using the Google Analytics opt-out browser add-on. You can set your browser to refuse cookies or to alert you when cookies are being sent. If you disable cookies, some parts of the Site may not work properly.

7. Data retention

We keep personal information for as long as needed to fulfill the purposes described in this policy, which for business inquiries generally means for as long as we have an active relationship or a reasonable business interest in following up, and thereafter as required by law or for legitimate recordkeeping. Lead information received from LinkedIn is stored in our CRM and retained on the same basis. You may ask us to delete your information at any time (see Section 9).

8. Security

We use reasonable administrative, technical, and physical safeguards designed to protect personal information from unauthorized access, use, or disclosure. No method of transmission over the internet or electronic storage is completely secure, so we cannot guarantee absolute security.

9. Your privacy rights

Depending on where you live, you may have the right to request access to the personal information we hold about you, to correct it, to have it deleted, to receive a copy of it, to opt out of certain uses, and to not be discriminated against for exercising these rights. To make a request, contact us using the information in Section 11. We will verify your identity before responding and will respond within the time required by applicable law.

California residents. Under the California Consumer Privacy Act (CCPA) you have the rights described above. In the preceding twelve months we have collected the categories of personal information described in Section 2 (identifiers, professional information, and internet activity information) for the purposes described in Section 3, and disclosed them to the categories of recipients described in Section 4. We do not sell personal information and do not share it for cross-context behavioral advertising within the meaning of the CCPA other than through the advertising measurement described in Section 6; you may opt out of that activity by contacting us or using the platform controls described above. We do not knowingly collect personal information from individuals under 16.

Residents of other states. Residents of states with comprehensive privacy laws (including Florida, Colorado, Connecticut, Virginia, Texas, Oregon, and others) may exercise comparable rights by contacting us. You may appeal a decision on your request by replying to our response.
