Anatomy of a Partnership

Anatomy of a Partnership

As you probably already know, Skilled Wound Care partners with nursing facilities to bring wound care to the elderly. We pride ourselves on our ability to partner with, educate, and learn from the nurses who work at these facilities. But what does such a partnership actually look like? Here is a brief look at how partnerships work.

The patients in nursing homes and other skilled nursing facilities are a population that is very prone to developing wounds (or having wounds that get worse). This could be because of diabetes, surgery, or physical immobility that prevents the patient from getting up or turning themselves.

When these patients develop wounds, there are a few ways that the facility could handle it. Of course, there is always the option of a hospital visit, but that runs the risk of infection, or additional complications due to the stress of transportation. The other option is to bring in the wound care experts.

Skilled Wound Care is a mobile surgical practice, thus giving us the freedom and capability to tend to patients in the comfort of their their own room. We can provide bedside debridements, which give wounds a much higher chance of healing properly than other methods. Patients don’t need to undergo the stress of being moved, like when they are sent to the ER, and we can monitor the status of their wounds every week and adjust our treatment plan to help them heal as quickly and effectively as possible.

When we partner with a facility, we become part of their team. Their patients’ wounds are our responsibility, and we provide the very best care, expertise, and treatment that we can. But that partnership is not complete without the nurses: a unique part of Skilled Wound Care’s approach is our focus on actively involving facility nursing staff in wound care. We are teachers and healers, educating nurses on how to better treat wounds, how to use better measurement and treatment protocols, and how to make sure that the plan we put in place is carried out through the week. We know that that teamwork is the recipe for truly successful practice: it takes a team of highly skilled, competent individuals to heal a sophisticated wound, and we’re lucky to work with such outstanding partners..

Treating wounds and sharing knowledge is just the beginning of a partnership with us. When we treat wounds at any facility, the number and frequency of hospital visits or re-admissions decreases. Thus, costs are also lowered, and patient satisfaction improves.

Skilled Wound Care’s first goal is care for the patients--but that’s only the beginning.

Come partner with us! Let’s heal some wounds together.

Skilled Wound Care is a mobile surgical practice committed to transforming the chronic wound care model in nursing facilities. Wound care experts make weekly bedside visits to patients in long-term care facilities, avoiding transfers to hospitals or clinics. Our expert physicians give patients the most up-to-date and effective wound treatments, and educate facility staff on how to help patients continue to heal quickly and effectively between visits. This model of collaborative care allows SWC’s physicians to improve patients’ lives and health outcomes, to empower nursing staff, and to raise public awareness. Skilled Wound Care, along with its nurse and nursing home partners, is working every day to positively transform traditional nursing home wound care.
