This 2019 is an exciting time for us at Skilled Wound Care and an opportunity to provide even more enhanced services for our nursing home partners. This year, I am excited to announce as one of our many new wonderful programs: The WONDER Program.
WONDER PROGRAM:
Woundcare Optimization with Nurses for Decreasing Excess Readmissions
What is the WONDER PROGRAM you may ask:
As of October 1, 2018 SNFs are exposed to a 2% downside reimbursement adjustment from CMS for having a higher than benchmark readmission rates. The WONDER Program will work as a partnership with our nursing home partners to prevent Wound and G-tube readmissions!
Our comprehensive wound and G-tube management program involves a close partnership between SWC clinicians and your Treatment Nurses during the 30-day period after discharge from a hospital. It supplements our already extensive wound care services by providing the following augmented care free-of-charge to you:

Additional monitoring for residents within 30 days of discharge from a hospital.

On-call access to your SWC wound care provider during the 30-day post-hospital-discharge window for wound care related services.

Where residents are physically and cognitively able, education on prevention of exacerbation of their wounds.

Education to your nurses on symptoms to watch out for in the 30-day window, and timely appropriate interventions.

Outcomes tracking for all readmissions.
