DFU Risk Education
Identify and educate patients at risk for the development of diabetic foot ulcers (DFUs).
Patients may not understand their risk factors or the need for preventive care. Providers must engage patients to attend preventive foot care visits for their diabetes.
DFU Prevention - Proper footwear eval
A foot examination documenting relevant vascular, neurological, dermatological, and structural / biomechanical findings.
Counseling on appropriate footwear should be based on the evaluation.
Care Plan Documented / Discussed
Patients 65 and older who have an advance care plan or surrogate decision maker documented in the medical record.
or
Documentation that an advance care plan was discussed but the patient did not wish to provide an advance care plan.
